One time registration fee:

Non-Member Offer

  • For a limited time, Dr. Booth will be offering non-membership comprehensive care visits

  • Comprehensive care visit: sick visit, annual physical, and medical consultations

    • $325 per visit

    • 45-minute visit (Includes physical exam and review of medical history)

    • 1 - Follow up visit (via telemedicine) to discuss labs/imaging (Must be completed within 30 days of initial appointment)

    • Prescription for a 30-day supply of medication if deemed necessary by physician

    • Medication, Labs & imaging not included
